The Ambrose Health Testing Kit is intended for use in the non-invasive collection of saliva samples. Human DNA from the Saliva sample is isolated, stabilized and suitable for use in FDA cleared molecular diagnostic applications. To ensure accurate results, patient should ensure that the sponge tip does not come in contact with any other surface, prior to collection.

1. Donor should not eat, drink, smoke or chew gum 30 minutes before collecting saliva sample.
2. Rx only. For personal use.
3. Read all instructions carefully prior to sample collection; deviation may lead to inadequate sample and impact DNA yield.
4.Choking hazard. Caution should be used when inserting sponge into the mouth.
5.Donors with Xerostomia (dry mouth) may not collect adequate sample using the instructions resulting in lower DNA yield and an invalid sample for use.
6.Wash with water if stabilising liquid comes in contact with eyes or skin. Do NOT ingest.

How to correctly use the swab

Please follow instructions carefully to properly collect DNA samples. Call for help if you run into any issues while attempting these steps.

1.

Open package, remove collector and start swabbing

Open package and remove collector without touching sponge tip. Place sponge as far back in the mouth as comfortable and rub along lower gums (see image in g) in a back and forth motion. Gently rub gums 20 times. If possible, avoid rubbing teeth.

2.

Repeat rubbing 20 times on lower gum.

Gently repeat rubbing motion on opposite side of the mouth and lower gums for an additional 20 times.

3.

Unscrew cap from collection tube.

Hold the tube upright to prevent stabilizing liquid inside from spilling. Unscrew cap from collection tube without touching the sponge.

4.

Insert sponge and secure cap tightly.

Turn the cap upside down, insert the sponge into the tube and close cap tightly.

5.

Invert closed tube and shake vigorously.

Invert the capped tube and shake vigorously 20 times.

What to do when I’ve completed these steps?

Place your sample back inside the pre-paid back box, along with a signed copy of your patient consent form and drop it in your mailbox for USPS pickup.
